I consent to my child participating in online instrumental tuition with Merton Music Foundation through the medium of live video lessons/rehearsals via the video conferencing platform Zoom. I have read and understood the Online Learning Code of Conduct and agree to adhere to it.

Photo Consent

Photographs and/or video recordings may be taken of your child whilst participating in Merton Music Foundation activities and we are seeking your permission to use such images/recordings for information and promotional purposes in flyers and displays as well as on the Merton Music Foundation website and social media pages.

In accordance with the Data Protection Act 1988, we are seeking your permission before we can photograph or make any recordings of your child so please fill out the form below.

I hereby agree to my child being photographed/filmed by Merton Music Foundation for promotional purposes. I understand that the photos/videos may be used in digital and printed publicity materials, including displays and exhibitions.
